When the death occurs, the certifying Doctor will issue the Medical Certificate of Cause of Death (MCCD), this is passed to the Medical Examiner (ME) who will scrutinize the MCCD, question the certifying Doctor and providing the ME is satisfied of cause of death, the ME will contact the family and ask a few medical questions. If the family are content with the cause of death, the MCCD will be sent to the Registrar who upon receipt will contact the Informant or Next of Kin (the person responsible who will register the death) and invite them to the Registry Office to register. Your Funeral Director is not permitted to register a death.

The registrar also supplies a service called ‘Tell Us Once’ which lets you report a death to most government organisations in one go. Please speak to the Registry Office for more information.
Documents that will be issued at Registration
- Certified Copy of an Entry in the Register (Death Certificate). There is a standard fee for each copy. If additional copies are required at a later date, the fee per copy may double. This certificate is not usually needed by the Funeral Director, but you may need it for other organisations, such as banks, probate, insurance companies and pension providers.
- Certificate for Burial or Cremation (Green Form) and the Form 9 (which will be attached). This needs to be given to the Funeral Director before the funeral can take place. The Form 9 will also state whether any hazardous implants are present. If a cremation is taking place, the Funeral Director will arrange removal of these devices.
The Coroner
On some occasions the Coroner may be involved, and a slightly different procedure will follow, we will advise accordingly, especially if there will be an inquest.
